Delayed recognition of the mechanics of aerosol transmission – the primary mode of spread of SARS-CoV-2 – at the beginning of the COVID-19 pandemic led to inadequate early recommendations regarding nonpharmaceutical interventions and poor initial messaging about how to reduce infection risk. We highlight key epidemiological details of Andes virus (ANDV) transmission on the MV Hondius cruise ship and during prior reported outbreaks with confirmed person-to-person spread in Argentina, which, together, suggest aerosol spread may be a key mechanism of transmission of ANDV between source cases and susceptible contacts in permissible environmental settings.

Opening doors improves in-room ventilation and promotes aerosol leakage, revealing a trade-off between the dilution and contamination of shared spaces. Architectural elements such as privacy curtains contribute to airflow stagnation and uneven aerosol removal. Effective infection control strategies must incorporate airflow pathway management and localized filtration to prevent unintended aerosol migration in mechanically ventilated healthcare settings.

😱 Extreme weather breaks MRI scanners and cooling units and workload rises for sleep-deprived staff on sweltering wards ‘Infection control becomes almost impossible’: four doctors on the NHS heatwave crisis

Several NHS trusts in England have declared critical incidents as a direct result of the extreme heat. One hospital had done so after its machines failed in multiple areas, a doctor said. Labs used for testing were affected and two linear accelerator machines, used to treat cancer patients, had stopped working amid the high temperatures.

The doctor said that although they were working in a relatively new care setting, it was “tacked on to an old Victorian hospital”, creating severe infrastructure challenges. “It’s hopeless, really,” they said.

The doctor also said their NHS trust had faced “major issues” with IT servers overheating on Wednesday. “We thought we were going to lose everything, so we were all asked to turn off non-essential computers and electrical equipment, including lights.”

Results:
Among 239,334 cases, the Omicron-dominant era (waves 5–7) accounted for 97.88% of the total burden. Although the mean hospital stay decreased from 12.6 days (wave 1) to 3.7 days (wave 5), mortality and multisystem inflammatory syndrome in children were concentrated during Omicron waves. Children 1–4 years of age experienced the most severe outcomes. Multivariate analysis identified cardiovascular disease (adjusted odds ratio [AOR]: 3.97), neurologic disorders (AOR: 2.88) and obesity (AOR: 2.68) as significant independent risk factors for severity. After adjusting for age and sex, vaccination showed limited protective effects against severe outcomes in this cohort.

2. When Omicron came along, we lucked out because this was a highly transmissible version of SARS-CoV-2, similar to measles virus, while primarily affecting the upper respiratory tract without causing severe illness in most cases. So, this was a blessing in disguise (in my opinion) and helped with hybrid immunity and in reducing the impact of the pandemic.

Another medical paradigm shift I'd like to see is an end to waving pneumonia around as a cause of sickness, hospitalization, death. I'm less certain in predicting this, though, as it's intertwined with the poopshow that is infectious disease medicine & infection control.

And yes, I know that study concludes aerosol transmission from lungs is uncertain but it was 2019, before the great leap in aerosol understanding catalyzed by SARS2. We know now - lung involvement--> exhaled infectious aerosols.
