That's why they should bring back the old 0-9 d20s! My first one was like this, and we just colored half the numbers red and that meant "add 10." 😂
or even a hilarious joke. Only stipulation was they had to write a brief summary of "why." I would read these out anonymously while we tallied up the XP. Great feedback mechanism for the other Players, and a window into what everyone was enjoying! I have **boxes** of index cards like this:

Like a "Heat" Mechanic? I think you could do this in a way by structuring Encounter Tables. Put some ominous signs lower down, but the "Big thing" up high. Then, apply a modifier to the rolls the longer you spend there/each visit. Placing it above the "upper bound" means it won't show up at first.
Add in the Classic Dungeons & Dragons Game. IIRC, there are some *slight* differences from the Denning Black Box.
I'm more a 2d6 Morale Check Referee (because I like how this resembles Reaction Rolls), but I did put together a little "scratch table" for some other options on Failure just to keep things "more interesting"

There are *subtle* changes in the D&D ones that I've thought far too much about. Instead of "Friendly" in Holmes Basic...Monsters might end up "Enthusiastic, volunteers help!" Which adds a completely different dimension:
The "Nested" one in BECMI is always fun, it creates a kind of *decision* tree/automated Monster Response over time which I always felt was interesting, because of the weight it applies to those "secondary" rolls: (Triangular Distribution is important with these for several reasons though)
Heh, I think I used a shot of *this exact one* a while back for a Magic Item I whipped together:
B/X characterizes them as a 12 HD Air Elemental, and OSE thoughtfully provides a Stat Block. Always found the Morale of 10 quite funny though! Imagine a Tornado fleeing Combat!

It's totally sweet. One of my favorite stories from AD&D was a DISCOVERY! result on the Potion Miscibility Chart. Ended up with a Fighter that could *Speak with Animals* Permanently!
