The first example is health and human services secretary Robert F Kennedy Jr freezing US funding to Gavi, the multilateral agency that finances vaccines for half the world's children.5 In fiscal years 2025 and 2026, the US Congress appropriated $600m for Gavi, but Kennedy is refusing to pay.5 He wants the agency to stop delivering multidose vaccines that use thiomersal,6 a preservative containing ethylmercury (thiomersal does not contain the neurotoxin methylmercury). Since 2005, Kennedy has falsely claimed that thiomersal is linked to autism,7 despite clear evidence since 2005 that no link exists,8 and extensive research confirming the safety of thiomersal in vaccines.9 Unless Gavi meets Kennedy's baseless demand to remove thiomersal from multidose vaccines, he will continue to freeze US funding for Gavi, meaning fewer children will get vaccinated and more will die. Fortunately, Gavi is refusing to give in to his demands.

A second example is the extractive aid contracts that the US is seeking with selected countries. In negotiations, the US offered the Zambian government up to $2bn in aid over the next five years, including funding for lifesaving antiretroviral medicines, but only if Zambia gives the US permission to mine its mineral resources.12 The 1.3 million people living with HIV in Zambia are being used as a bargaining tool. If Zambia does not agree to US demands, people will lose access to antiretrovirals, become more ill, and likely die. Zambia has rightly rejected the deal.13 Ghana and Zimbabwe rejected similar deals that would have given these nations health aid in exchange for access to their sensitive health data.14
