Ranking exercise parameters for Achilles Tedninopathy (AT) 🥇🥈🥉 For rehab of mid-portion AT contraction intensity appears to be key. With insertional AT dorsiflexion range may play a more prominent role. We still want to load progressively but we often start out of painful, dorsiflexed positions.
I teamed up with @Liz_Bayley_Physio to create this progressive rehab programme for Plantar Heel Pain. We've called it 'LiTHE' (Liz and Tom's Heel Exercises) and it's aimed at active and sporty patients. It's not a recipe though so it's best to adapt it to suit individual needs.
Posterior knee pain differential diagnosis. I’ve focused on the more common injuries but it’s worth noting that bone stress injuries (such as tibial stress fractures) and bone tumours can also cause posterior knee pain, although they are more rare.
Progressing rehab can be challenging if symptoms remain irritable. Here's an approach that's often effective and builds from what's manageable towards individual goals.

Choose your words with care! Patients tend to interpret phrases like ‘degenerative’ and ‘wear and tear’ very negatively. Check their understanding too - one study found that some patients thought a knee ‘effusion’ meant the joint was fused together! 🤯
Muscle activity during running 🏃🏽♀️ The quads, calf and glutes are important shock absorbers and the hamstrings work hardest during the swing phase.

Cross-training is a great way for injured athletes to maintain fitness. Look for options that are comfortable in terms of symptoms and easily accessible for the patient.

Under fuelling can impair performance and increase injury risk. Carbs are a vital source of energy for athletes. Eating 3 meals per day and building a routine of fuelling before, during and after training can help ensure energy needs are met. Speak to a Sports Nutritionist for tailored advice.

Training Stress AND recovery. Two vital ingredients for athletes to progress. You need both!
Running with excessive forward trunk lean can affect efficiency and increase load on the Glutes and Hamstrings. These cues can help! 🏃
Intrinsic foot muscles can be weak in patients with Plantar Heel Pain🦶 Progressive strengthening of intrinsic and extrinsic muscles can be effective, especially in runners and active individuals. See my previous posts for recent research on which exercises are effective to target these muscles.
It's recommended that we load at ≥70% of 1 Rep Max during rehab but what does this translate to in terms of reps to fatigue? Nuzzo et al. (2024) suggests 70% of 1RM is ~15RM which is a good place to start with loading & can be progressed towards 6RM (& heavier) as symptoms allow
Many different loading programmes can be effective for tendinopathy. The most effective ones tend to be progressive and tailored to the individual with rehab adapted based on symptom response 🏋️

Kinesiotape or low dye tape for Plantar Heel Pain? Both strategies may be effective for short term pain reduction in PHP. Consider patient preference, goal activities and previous response to taping.
Neural mobility can be reduced following back pain, hamstring injury or sciatica. A gym ball nerve glide is a nice option to help restore this movement and prepare for running 🏃🏽♀️ ⚠️Be sure to adapt to individual need so it’s well tolerated, especially if symptoms are irritable.
Always consider bone health in patients presenting with a stress fracture. Unless these factors are addressed there's a high risk of future bone stress injuries. Multidisciplinary input may be needed for a full assessment of an athlete’s needs.
🔟 Hamstring exercise options. Each should be adapted to meet individual needs, especially sprinting which needs a gradual introduction at the right stage of recovery 🏃🏽♀️
