The primary challenge is the counterfactual. Medicine interferes with Pt trajectories, as everyone is treated. We need to check whether the treatment differs between groups, then infer whether that treatment difference is meaningful and positive. Often too much noise to pick up a signal from CDSS.
I have sympathy for Scimago having to deal with all those messages from confused people trying to submit papers to the wrong journal, but hosting ads like this does a lot to make that sympathy evaporate #academia

Basically, fit the model with chains = 0, then create an update() function with recompile = FALSE and the sampling specs you want. I've usually had success with {{furrr}} for the parallelisation part. Can't take full credit for this one as I needed an LLM to show me how the recompile step worked.
