I know Sir Terry was up on his SFF, so I wonder if he read James White's "Star Surgeon" (1963). Most of White's stuff is optimistic hospital cooperation with various BEMs, but this one involves an interstellar war brewing and a humorless Space Corps officer. When tensions boil over, we get this:

You may have noticed that fantasy decor uses a lot of skulls. Well, some of those are battle trophies, I'm sure, but for most of your skull needs you can just get them from the Goblin Market!
