Governor Arihnda Pryce from Star Wars Rebels, who blew up a fuel depot in an attempt to kill a handful of rebels. She managed to get a Jedi, which is more than the Russians did, but this led to the fall of the Empire at Lothal, the disappearance of Thrawn, and the first major victory for the Rebellion. Basically her fuckup was the beginning of the end for the Empire.
