What have we learned? Key areas for improvement include communications, risk assessment, interactions between parents, centres, schools, documentation, per-existing injuries, fit between participants and activities, policy and procedures, consideration of equipment and environment
These incident contributory factor can be mapped out into the 'Big Eight' contributory factors., with interaction between all of the elements. (Does your risk assessment take these interaction into account?)
The most frequently reported incident contributory factor at each system level shows that there is a high level of communications problems, paired with policy/procedure issues at upper management. This outcome would not surprise people in the field.
